Job summary

Jefferson University Physicians seeks a compassionate Palliative Care Social Worker to support patients and families across inpatient, ambulatory, and home settings. The role partners with palliative care physicians, nurses, chaplains, and other professionals to address emotional, social, and practical needs of patients with serious illness.

The social worker assesses coping, develops discharge plans, and provides crisis counseling.

Qualifications

• Master’s Degree in Social Work from an accredited program.
• Minimum 2 years of hospital-based social work experience.
• Knowledge of medical terminology and resources.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Develop and implement discharge plans with the care team.
• Provide counseling to patients, families, and significant others.
• Coordinate with multidisciplinary team to support end‑of‑life and palliative care needs.
