Job summary

Intermountain Health in Billings, Montana is seeking a Palliative Care RN to provide compassionate, patient- and family-centered care. You will coordinate care, educate families, and collaborate with an interdisciplinary team to relieve symptoms and support advance care planning.

Qualifications

• Current RN license in the state of practice; compact license must transfer within 60 days.
• BSN from an accredited program; education will be verified.
• BLS certification for Healthcare Providers.
• RNs with <12 months of experience must obtain BSN within four years of hire.

Responsibilities

• Coordinate care and serve as a navigator for palliative patients and families.
• Educate patients and families on diagnosis, options, and resources.
• Assess symptoms and support symptom management plans with providers.
• Triaged calls, questions, and care concerns from patients and families.
• Coordinate services across primary care, specialty care, hospice, home health, and community resources.
• Support advance care planning conversations and documentation.
• Provide ongoing follow-up and care coordination throughout the journey.
