Qualifications

• Must have a valid NYS Medical License.
• Board certification in Palliative Care is essential.
• Experience in managing complex patient cases is preferred.

Responsibilities

• Provide holistic, patient-centered care with a dedicated team.
• Manage pain, nausea, fatigue, and other symptoms for serious illnesses.
• Facilitate conversations with patients/families about goals and advance care planning.
• Develop care plans tailored to individual patient needs.
• Collaborate with interdisciplinary teams for seamless continuity of care.
• Educate and support patients and families during illness and end of life.
• Participate in quality improvement initiatives for palliative care delivery.
