Job summary
Intermountain Health in Utah is seeking an Advanced Practice Practitioner for Palliative Medicine Home Services. You will provide compassionate care to patients in their homes, traveling across the Salt Lake Valley, Park City, and Tooele areas.
The role features a standard schedule of Monday through Friday, 8 am to 5 pm, with competitive compensation and a strong focus on quality patient experience. A valid APRN/PA‑C, board certification, and Utah license are required, along with prescriptive
Qualifications
- APRN or PA-C from an accredited program.
- Board certification.
- 2+ years of experience in specialty medicine; palliative care preferred.
- Active, unobstructed Utah medical license, or the ability to obtain one.
- Full prescriptive authority and an active DEA, or the ability to obtain one.
- Ability to complete Intermountain Health's credentialing process.
- We are not able to accept candidates requiring Visa sponsorship at this time.
Responsibilities
- Provide palliative care to patients in their homes as part of a care team.
- Travel to patient homes across Salt Lake Valley, Park City and Tooele Areas.
- Work Monday through Friday, 8am-5pm.
