Job summary

TeamHealth is seeking a quality-driven palliative care nurse practitioner or physician assistant to join our post-acute care team in the South Nashville area. This full-time role (5 days/week) involves rounding in skilled nursing facilities, with strong support, autonomy, and earning potential.

Compensation is fee for service with an annual range of $109,595 to $131,514 and no cap on productivity income. Night call coverage is provided to support work-life balance and professional growth.

Qualifications

• Active NP/PA license in TN with DEA registration.
• Minimum 2 years in palliative care.
• Strong regulatory knowledge and clinical foundation.
• Excellent communication and teamwork.

Responsibilities

• Perform comprehensive patient assessments, including medical histories and physical exams.
• Develop, implement, and adjust individualized care plans based on patient needs.
• Monitor progress and coordinate care to optimize outcomes.
• Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach.
• Educate patients and families on health management and preventive strategies.
• Maintain accurate, timely medical records in compliance with regulations.
