Job summary

TeamHealth is seeking a quality-driven palliative care nurse practitioner or physician assistant to join our post-acute care team in Greeneville, Tennessee. This full-time position (5 days/week) involves rounding in skilled nursing facilities with autonomy and strong earning potential.

You will provide high-quality palliative care to a diverse patient population, collaborate with an experienced multidisciplinary team, and support patients and families during pivotal moments.

Qualifications

• TN NP or PA license and at least 2 years in palliative care.
• Strong clinical foundation and knowledge of regulatory standards.
• Excellent communication and interpersonal skills.

Responsibilities

• Perform comprehensive patient assessments with histories and exams.
• Develop, implement, and adjust individualized care plans.
• Monitor progress and coordinate care to optimize outcomes.
• Collaborate with physicians, nursing staff, and other healthcare professionals.
• Educate patients and families on health management and preventive strategies.
• Maintain accurate medical records in compliance with regulations.
