HealthDrive Corporation is seeking a compassionate full-time Palliative Care Nurse Practitioner to provide comprehensive palliative care in skilled nursing and long-term care settings. Partner with residents, families, and interdisciplinary teams to align care with goals and improve quality of life.

The role emphasizes goals-of-care discussions, symptom management, and reducing unnecessary hospitalizations while supporting families and staff in a patient-centered approach.

Qualifications

• Master’s degree in nursing or equivalent.
• State NP license required.
• Board certification required.
• DEA license required for controlled substances.
• Valid driver’s license.
• Ability to travel to facilities in assigned territory.
• Ability to work independently and with on-site teams.
• Proficient with email, internet, and healthcare software.

Responsibilities

• Conduct comprehensive palliative care assessments for residents with serious illness.
• Manage symptoms and adjust treatment plans.
• Lead goals-of-care discussions with residents and families.
• Coordinate medication regimens and deprescribing when appropriate.
• Collaborate with pharmacists and facility clinicians to improve outcomes.
• Maintain timely, compliant clinical documentation in the EMR.
