Bon Secours Mercy Health is seeking an Experienced Palliative Care Nurse Practitioner to join our team in Portsmouth and Suffolk, VA. This full-time role offers 4x10s with no call and involves inpatient consults at Maryview Medical Center, with potential outpatient clinic coverage at Harbour View Palliative Care Clinic in Suffolk.

You will work with an interdisciplinary team to provide symptom management and goals-of-care discussions.

Qualifikationen

• 3–5 years of practice in palliative care, critical care, or hospice care.
• MSN with Virginia licensure or ability to obtain licensure and board certification as FNP/ACNP/AGNP.
• BLS certification is required.
• Demonstrates emotional intelligence and a patient-first mindset.

Aufgaben

• Provide expert palliative care services focusing on symptom management and quality of life.
• Collaborate with interdisciplinary team to deliver holistic patient-centered care.
• Conduct goals-of-care conversations and advance care planning.
• Lead discussions on care goals, advance directives, and treatment preferences.
• Coordinate with physicians, social workers, chaplains, and others to create care plans.
• Assist in transitioning patients to hospice when appropriate.
• Utilize EPIC/MyChart for documentation and care coordination.
• Contribute to a warm, patient-centered environment emphasizing dignity and communication.
