Job summary

Qualifications

• Minimum of 3 years experience working with patients with chronic and life threatening illnesses.
• Excellent communication and interpersonal skills.
• Knowledge about home care, palliative care, hospice and discharge planning.
• Board certified in Palliative Medicine within one year of hire.
• Master's degree preferred in nursing or social work.
• NJ State professional licensure required: RN or SW/LCSW.

• Board certified in Palliative Medicine within one year of hire.

• Knowledge of palliative care interventions strongly preferred.
• Basic skills in outcomes research strongly preferred.

Licenses and Certifications Required:

• NJ State Professional Registered Nurse License or NJ Licensed Social Worker or NJ Licensed Clinical Social Worker.
