Job summary

OSF HealthCare is seeking a Palliative Care Advanced Practice Provider (App/APRN or PA) to join an inpatient team in Illinois. The APP will assess, diagnose, and manage symptoms for patients with serious illnesses, collaborating with physicians, nurses, and other providers to develop and execute comprehensive care plans.

The role requires MSN/MPAS, IL licensure or eligibility, and BLS, with DEA and IL controlled substances credentials preferred.

Qualifications

• MSN or MPAS required.
• Current IL license or eligibility.
• BLS certification required.
• DEA registration and IL controlled substances license required.

Responsibilities

• Provide specialized palliative care services, including symptom management and advance care planning.
• Assess patients, diagnose, and implement treatment plans for complex conditions.
• Collaborate with interdisciplinary palliative care team to address holistic needs.
• Maintain accurate medical documentation and coordinate care across inpatient, outpatient, home, and long-term care settings.
• Communicate with patients and families to promote shared decision-making; participate in quality improvement initiatives.
