Qualifications

• Must be a graduate of an accredited US medical school (MD or DO).
• Board Certified or Board Eligible in Pain Medicine (via Anesthesiology, PM&R, or Neurology subspecialty pathway).
• Active and unrestricted Indiana medical license (or eligible within 90–120 days).

Responsibilities

• Provide evaluation, diagnosis and treatment for adult pain conditions.
• Perform interventional pain procedures in outpatient and procedure settings.
• Collaborate with spine surgeons, neurologists, orthopedists, and physical therapists.
