• Licensed NP or PA in Wyoming.
• Experience and interest in palliative and pain management.
• Comfortable providing palliative and pain management in ALF/SNF settings.

Responsibilities

• Assess patients in ALF/SNF environments.
• Provide palliative and pain management to optimize patient outcomes.
• Perform pain management procedures as needed.
• Define and document patient diagnoses.
• Prescribe and refill medications as appropriate.
• Educate patients and families and manage care plans.
• Record findings and treatment courses in charts and follow-up notes.
• Engage in departmental activities to improve training and outcomes.

Compass Palliative Care, a leading California-based palliative care provider, seeks outstanding Nurse Practitioners/Physician Assistants with experience and interest in palliative andpainmanagement. Our practice provides palliative andpainmanagement appointments for California-based patients. Our practice provides palliative andpainmanagement services in the ALF and SNF settings in the Casper area.

The practitioner will be assigned a specific patientloadfor which they will be responsible. Our program is run by experienced and board-certified physicians who take pride in providing training and in-service education to their staff. They believe in the advancement of their staff as well as creating the best possible outcomes for the patients they serve.

Responsibilities include, but are not limited to:

• Assess patients in an ALF/SNF environment
• Provide palliative andpainmanagement to treat the patient best
• Performpainmanagement procedures as needed
• Define and document patient diagnoses
• Prescribing and refilling medications
• Patient and family education and management
• Notate findings and treatment course in patient charts, including follow-up notes
• Activelyparticipatesin departmental activities to improve training methods and achieve company goals

Requirements:
