U.S. Department of Veterans Affairs in Washington, DC seeks a Clinical Pharmacy Specialist in Pain Management to support chronic pain management in high-risk patients on opioids.

The CPP will co-manage patients with the pain clinic and primary care providers, specializing in drug therapy management and monitoring for chronic non-malignant pain. The role includes patient assessment, lab monitoring, drug info to the care team, supervision of residents, and coordination with the Pain Management

Qualifications

• Education recognized by the U.S. Department of Education; foreign education requires a Certificate of Foreign Equivalency.

Responsibilities

• Monitors medication therapy for pain management in chronic pain, including initial assessment of new patients and follow-up in clinics.
• Makes recommendations to optimize drug therapy, prevent adverse events, and reduce medication errors.
• Provide drug information to physicians, nurses, trainees, and other health care professionals; participate in interdisciplinary care conferences.
• Supervise and precepts residents and students; present lectures; participate in research and coordinate resident projects.
• Act as liaison between Pain Management, primary care, and outpatient pharmacy; participate in quality improvement activities.
