Job summary

LiveWell Homecare Agency, LLC is seeking a registered nurse to provide direct patient care including assessment, diagnosis, planning, implementation and evaluation within established nursing standards.

Key responsibilities include promoting patient- and family-centered care, educating on illness management and medication administration, developing and documenting individualized care plans, and ensuring safety and regulatory compliance.

Qualifications

• Minimum education: Nursing Diploma/Associate’s Degree; preferred Bachelor’s Degree in Nursing.
• RN licensure required.
• At least 2 years of nursing experience is preferred; no supervisory experience necessary.

Responsibilities

• Provide direct patient care activities including assessment, diagnosis, planning, implementation, and evaluation within established nursing standards.
• Promote patient- and family-centered care in a healing environment, educating on illness management, post-treatment care, and medication administration.
• Develop, implement, and document individualized care plans with clear goals in collaboration with the healthcare team, patients, and families.
• Assess physical, psychosocial, and emotional needs, considering patient preferences, values, emotional, spiritual, cultural, and population-specific factors.
• Work in a safety-sensitive environment, maintaining constant alertness and safe practices.
