Job summary

Diversity Group is seeking a Staff Registered Nurse to join the Patient Aligned Care Team (PACT) in an outpatient setting to deliver gender-specific, evidence-based care to primary care patients and oversee licensed nursing staff. The RN will provide direct Veteran care and coordinate with the care team in the Primary Care Department.

The role emphasizes care coordination, patient education, and collaboration with the interdisciplinary team to optimize outcomes.

Qualifications

• English language proficiency is required.
• Graduate of a nursing program approved by a recognized accrediting body.
• Active registered nurse license in a US state or territory.
• Foreign-trained nurses require degree equivalency validation per VA standards.
• BSN degree preferred; outpatient experience is helpful.

Responsibilities

• Provides direct Veteran care and care coordination within the Primary Care Department.
• Manages assigned patient panels and ensures appropriate access.
• Offers health promotion, education, and coaching on wellness and chronic disease management.
• Directs nursing education, orientation, competencies, and quality improvement initiatives.
• Collaborates with the interdisciplinary team to influence patient outcomes.
• Administers medications and procedures per policy and guidelines.
