The US Department of Veterans Affairs’ Clearlake VA Outpatient Clinic seeks a Licensed Vocational Nurse (LVN) to join the Patient Aligned Care Team (PACT). The LVN provides direct patient care and supports coordination of services within the PACT model to promote continuity of care for Veterans.

Applicants must meet basic requirements including US citizenship, English proficiency, and active LVN licensure; education from an approved school, and licensure in the state.

Formación

• Graduate of a school of practical or vocational nursing approved by the State or NLNAC, or equivalent military education accepted for licensure.
• Active, full, current, and unrestricted licensure as a Graduate LVN/LPN in a U.S. state or D.C.

Responsabilidades

• Provide direct patient care within the PACT model.
• Support coordination of services to promote continuity of care for Veterans.
