The U.S. Department of Veterans Affairs is seeking a Licensed Vocational Nurse (LVN) to join the Patient Aligned Care Team (PACT) in the Sorrento Valley, CA clinic.

The role focuses on direct patient care within a complex healthcare system that spans inpatient and outpatient services for Veterans. Responsibilities include specimen collection, medication administration, EHR documentation, and patient education.

Responsabilidades

• Perform LVN/LPN duties within the Patient Aligned Care Team (PACT) at the North County San Diego Community Based Outpatient Clinic in Sorrento Valley, CA.
• Obtain/collect specimens as ordered and provide direct patient care and support; recognize changes requiring intervention.
• Complete EHR documentation and sign patient activity within required timelines and protocols.
• Administer medications and treatments to patients with complex nursing needs; assist with care coordination.
• Educate patients on medications, procedures, discharge instructions, and disease management; maintain patient confidentiality.
• Collaborate with Interdisciplinary Care Team; participate in performance improvement activities and staff education.
• Participate in morning huddles, staff meetings, and provide in-services as needed; document patient responses and outcome progress.
• Perform bedside point-of-care testing (e.g., nasal swabs, pregnancy tests) and respond to urgent or emergent situations.
