The San Francisco VA Medical Center is seeking a Licensed Vocational Nurse to provide direct patient care within the PACT team. The LVN delivers safe, compassionate nursing care and supports coordination of services for Veterans in a fast-paced outpatient setting.

Responsibilities include obtaining vital signs, assisting with examinations, administering medications and immunizations, performing procedures, maintaining infection control, documenting care, and coordinating follow-up appointments

Qualifications

• Graduate of a school of practical or vocational nursing approved by the state agency and/or NLNAC.
• Active, full, current, unrestricted licensure as a Graduate LPN/LVN.
• English language proficiency and U.S. citizenship requirements.
• Ability to work under supervision in a team-based outpatient setting.

Responsabilités

• Obtain and document vital signs and patient information.
• Assist providers with exams, procedures, and treatments.
• Administer medications and immunizations per orders and protocols.
• Perform wound care, dressing changes, and basic labs/ECGs.
• Maintain infection control and a safe clinical environment.
• Coordinate care within the PACT team and schedule follow-ups.
• Document care in the electronic health record per VA policy.
• Educate Veterans and families about care plans and medications.
