Job summary

The San Francisco VA Medical Center is seeking a Licensed Practical Nurse (LVN) to provide direct patient care within a team-based primary care model. The LVN will deliver compassionate, patient-centered nursing care while supporting care coordination within the PACT framework and ensuring timely access to services for Veterans.

Responsibilities include vital signs, assisting with examinations, administering medications, performing procedures, and maintaining high standards of infection control.

Qualifications

• Active, full, current LPN/LVN license in the United States
• 6+ months to 1+ year experience in nursing care settings (GS-3 to GS-6 requirements)
• Ability to work in a team-based primary care model and coordinate patient care

Responsibilities

• Obtain and document patient vital signs and symptoms
• Assist providers with exams, procedures, and treatments
• Administer medications and immunizations per orders
• Maintain infection control practices and a safe clinical environment
• Coordinate patient care activities within the PACT team
• Schedule follow-up appointments and referrals
• Document care in the electronic health record per VA policy
