Job summary

Qualifications

• Must be a graduate of a professional nursing program approved by ACEN/CCNE.
• Must hold a current, full, active RN license in a US state or DC.
• BSN preferred for PACT roles.
• Foreign-educated nurses may require degree equivalency validation.

Responsibilities

• Works with a patient panel and care team to manage chronic conditions.
• Uses nursing process and evidence-based practice to develop patient-driven care plans.
• Collects data and applies evidence-based changes to practice.
