Job summary

Valir PACE LLC in Oklahoma City seeks a dedicated social services assistant to coordinate community resources and support plans for participants, working closely with MSWs and the PACE team to ensure comprehensive care.

The role includes setting up life-alert systems, documenting care plans, and liaising with families to facilitate transitions of care. A high school diploma and 3–5 years in social services are required.

Qualifications

• 3-5 years of social services experience.
• 1 year working with the frail and elder population.

Responsibilities

• Assist MSW with setup of life-alert systems and document appropriately.
• Find and obtain access to community resources for participants and families as directed by the MSW.
• Complete and document the “Plan B” to identify community support.
• Scan documentation into the chart as directed by the MSW.
• Facilitate family meetings and coordinate between participant, families and IDT as needed.
• Support other departments as directed by PACE Director.
• Perform other duties as assigned.

This role may also support other roles as needed, including quality initiatives focused on improving services delivered to PACE participants or arranging for transitions of care for PACE participants as directed by the MSW &/or PACE Director.Duties/Responsibilities:Serves as assistant to the MSW(s) assisting with set up of life-alert systems and following up with appropriate documentation in chart and participant communication.Finds and obtains access to community resources, as directed by the MSW, to support participants and their families.Responsible for completing and appropriately documenting the “Plan B” document, designed to facility the identification of community support available to the participant.Scans documentation into the chart as directed by the MSW.Facilitates family meetings or conversations with participants as needed and liaises between participant, families and IDT as needed.May provide support to other departments as needed and as directed by PACE Director.Any and all other duties and responsibilities as assigned.Education, Licenses, Certifications and Experience:3-5 Years of Social Services Experience1 Year working with the frail and elder populationHigh school diploma or equivalent.
