Job summary

NeighborHealth in the PACE Meals and Nutrition program is seeking a Registered Dietitian to conduct comprehensive nutrition assessments, develop individualized care plans, and coordinate interventions for participants with chronic conditions in a clinical nutrition setting.

The RD will oversee nutrition-related competencies for Dietary Assistants and Health Aides, collaborate with the interdisciplinary team, and ensure care aligns with policy, evidence-based guidelines, and NeighborHealth

Qualifications

BS or MS preferred
At least one year as a Registered Dietitian
RD eligible from an accredited institution
Obtains state licensure within 3 months of hire
Maintains State and National registration/licensure
ServSafe Certification (Massachusetts Allergen Training) preferred

Responsibilities

Completes nutrition assessments and implements care plans at enrollment, annually, and at status changes per policies and procedures.
Completes progress notes and updates care plan problems, goals, and interventions in a timely manner.
Provides chronic care management based on participant-centered goals and evidence-based practice.
Addresses high-risk nutrition issues including weight management and skin integrity per policies and procedures.
Collaborates with the participant/caregiver and IDT to develop and implement team goals and care plans.
Delivers culturally, linguistically, and age-appropriate nutrition counseling to participants and families/caregivers.
