Job summary

Neighborhood Healthcare seeks a Nurse Practitioner or Physician Assistant to serve as a Primary Care Provider at our PACE location in Riverside, CA. You’ll deliver participant-centered care within an interdisciplinary team, contributing to health outcomes and quality of life.

Responsibilities include comprehensive exams, diagnostic testing, home visits, and education for participants and families. Competitive benefits and a team-focused environment are offered.

Qualifications

• Master’s degree in nursing is required.
• California NP license in good standing is required.
• CA DEA licensure is required.
• BLS certification upon hire, maintained.
• Board Certification or eligibility within 12 months of hire.

Responsibilities

• Provide comprehensive primary care to PACE participants.
• Perform physical examinations and health assessments.
• Order, administer and interpret diagnostic tests.
• Conduct injections, immunizations, wound care, and infection management.
• Educate participants/families on therapies and lifestyle changes.
• Coordinate home visits and specialty care as needed.
• Maintain thorough EHR documentation and care plans.
