Job summary

Golden Valley Health Centers Central Valley PACE Center is seeking a Lead Patient Care Assistant to deliver patient care activities and support nursing staff in a home care setting.

Responsibilities include mentoring PCAs, communicating care needs to LVN/RN and Manager, assisting in training, performing COVID and influenza tests when assigned, and delivering DME to participants' homes while documenting in EMR. Schedule is Mon-Fri, 8am-5pm in Modesto, CA.

Qualifications

• Valid California Driver’s License with acceptable driving record and vehicle insurance.
• High school diploma or equivalent.
• Current unencumbered California licensure as CNA/HHA/EMT/Paramedic or clinical medical assistant certification.
• Current BLS CPR Card (American Heart Association).
• Prior personal care assistance experience or ability to pass internal competencies within six months.

Overview

Supports mission, vision, and values of Golden Valley Health Centers Central Valley PACE Center by acting as the Lead Patient Care Assistant and delivering patient care activities and tasks as assigned to assist the nursing staff.THIS IS A HOME CARE POSITION.

Schedule: Monday - Friday, 8am - 5pm.

Compensation:$24.31 - $25.53 an hour

Responsibilities

• Acts as the mentor to PCAI.
• Communicates patient care needs to the LVN, supervising RN and update Manager.
• Assists in training PCA’s and other staff.
• Perform COVID and Influenza test when assigned.
• May assist in Day Center and Activities departments when there is downtime.
• Deliver DME to PRT’s home and document in EMR.
• Assist with Home Safety Assessments when assigned only when there is downtime on the schedule.
• Flexible to work weekends when needed.
• Provides basic personal care assistance and assistance with any other activities of daily living across settings to assigned participants.
• Communicates effectively with participants, families, staff, other departments and agencies.
• Works effectively in a team setting.
• Documents treatments/procedures, observations, nursing care and patient activities on appropriate forms.
• Participates in teaching/learning process with others.
• Follows guest relations policies and procedures.
• Abides by all established policies, rules and regulations.
• Attends staff meetings and takes part in participant care planning.
• Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
• Communicates effectively with participants, families, staff, other departments and agencies.
• Works effectively in a team setting.

Requirements

Physical Demands

• Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.
• Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
• Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
• Must have high manual dexterity.
• Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.

Work Environment

• Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
• Subject to unpleasant odors.
• The noise level is usually quiet to moderate, but may at times be noisy and crowded.

Education/Experience

• High School Diploma or equivalent required.
• Current unencumbered CA licensure as a Certified Nurse Assistant, Home Health Aide, Emergency Medical Technician, Advanced Emergency Medical Technician or Paramedic; or certification from an accredited program for clinical medical assisting.
• Current BLS CPR Card certified by the American Heart Association.
• Prior personal care assistance experience, or ability to pass internal competencies within six (6) months of hire.
