Automatic Data Processing, Inc. in Virginia is seeking a Nurse Practitioner to manage PACE participants, provide primary health care and case management, focusing on prevention, wellness and chronic disease management in collaboration with primary care physicians.

The NP will work with the Medical Director to coordinate the 24/7 PACE Model of Care, perform histories and exams, prescribe within scope, educate participants and families, and support the interdisciplinary team in care planning.

Qualifications

• Graduate of accredited nurse practitioner program.
• License and current registration to practice as a nurse practitioner in Virginia; specialty certification in Adult, Family, or Geriatric practice.
• Minimum of one year’s experience with frail or elderly population.
• Compliance with State or Federal requirements for direct patient care staff.
• Medically cleared of communicable diseases and up-to-date with immunizations.

Responsabilités

• Performs health histories and physical exams.
• Evaluates participants’ complaints and orders diagnostic procedures.
• Interprets and follows up on lab results.
• Prescribes medications within scope of practice and licensure.
• Provides comprehensive, coordinated care in day health center, nursing home, or home.
• Coordinates and evaluates plan of care with the team.
• Works with Medical Director and staff to meet medical needs.
• Acts as liaison between community physicians and healthcare professionals.
• Requests consultations and educates participants and families.
• Participates in interdisciplinary team for plan of care development.
