Job summary

SAN-Ysidro-Health in Vista, CA is seeking an Activities Coordinator to oversee recreational activities for frail elderly participants at the Day Center. You will plan, coordinate and evaluate programs, supervise staff and volunteers, and ensure compliance with policies and regulatory requirements.

Under supervision of the PACE Therapy Manager and IDT Manager, you will develop and implement daily, monthly and special events, manage data in the EHR, and participate in community outreach while

Qualifications

• Be a certified activity coordinator or an occupational therapist, recreational therapist, or have completed/or willing to be certified as an Activity Coordinator/Director position and receive regular consultation from an occupational therapist, occupational therapy assistant or recreation therapist.
• Flexibility, initiative, creativity, excellent organizational and time management skills.
• Minimum one year of experience working with the elderly in a social or recreational program preferred.
• High School Diploma or GED.
• Bilingual abilities (English/Spanish/Arabic) preferred.

Responsibilities

• Plan a comprehensive program tailored to the needs, interests and limitations of frail elderly persons to promote health, independence and social skills.
• Evaluate participants’ status and establish goals and treatment plans for recreation therapy.
• Collaborate with social workers, therapists and CNAs to develop care plans.
• Develop and post a monthly calendar of events and coordinate outings.
• Supervise activity aides, volunteers and community providers; manage budget requests.
• Utilize an EHR system for data entry and develop care plans to enhance wellbeing.
• Conduct home, hospital and SNF visits as required and travel to other sites as needed.
