Job summary

The Center for Elders’ Independence in Oakland is seeking an Activities Coordinator to plan and implement therapeutic recreational programs for participants. You will supervise staff and volunteers, coordinate with interdisciplinary teams, and ensure activities promote autonomy and quality of life.

Responsibilities include developing monthly activity schedules, providing training to aides and volunteers, coordinating transportation and meals, and documenting participant progress for the care

Qualifications

  • Have two years of experience in a social or recreational program providing and coordinating services for a frail or elderly population within the past five years.
  • Holds a Certificate of Activity Leadership approved by the California Department of Public Health.
  • Has a High School diploma, GED, or equivalent.
  • Demonstrated ability to independently facilitate creative arts groups; to engage all participants in creative activities to some extent. Able to organize and lead participant outings
  • Experience and interest in working with the elderly; understanding of the most frequent psychosocial and behavioral health issues of this population.
  • Interest and ability to work in interdisciplinary team.

Responsibilities

  • Conducts assessments of the participant’s needs and interests for the development of the individual plan of care by the interdisciplinary team according to policy and regulations.
  • Conducts accurate and timely reviews of the participant’s plan of care according to policy and regulations.
  • Provides orientation, on-going training and oversight to Center Activity Aide and volunteers.
  • Develops a monthly schedule of activities that is diverse, reflects current trends in activity program and addresses the functional and cultural needs of the center participants.
  • Posts the schedule of planned activities in a central location that is accessible to participants, families, caregivers or authorized representatives of participants.
  • Solicits input from participants and/or caregivers in the planning of the recreational activity program.
  • In collaboration with the Activity and Volunteer Functional Manager, recruits and oversees volunteers to enhance the group and individual activities.
  • Coordinates with the transportation and nutrition departments and to provide meals and safe transportation to and from activities in the community (outings).
  • Meets independent and group needs of all participants and ensures that participants have time to engage in activities of their own choosing.
  • Facilitates variety of individual and group entertainment or arts & craft activities, special events, etc. to meet the needs of participants at different levels of functioning.
  • Documents and reports to the interdisciplinary team any observed changes in participant’s behavior, including ambulation, eating habits, or level and type of social interaction.
  • Leads the planning and implementation of new activity groups, including utilizing community organizations and various other resources such as involving community groups and schools to enhance the program and add diversity.
  • Prepares periodic reports and keeps appropriate records.
  • Ensures that the day center decorations are appropriate, stimulating and enhance the appeal of the therapeutic environment.
  • Responsible for ensuring Quality Indicator information is collected and documented on a monthly basis
  • Participates in CEI quality assurance performance improvement program (QAPI) as assigned.
  • Follows all agency policies and procedures and applicable licensing requirements.
  • Maintains a courteous, helpful and professional attitude on the job.
  • Complies with all agency training requirements. Maintains professional affiliations and any required certifications.
  • Fulfills all necessary other administrative requirements such as reviewing staff reports, generating reports, developing and keeping statistical records, reviewing chart entries, annual staff performance evaluations, etc.
  • Performs all other job-related duties in a positive and helpful manner as assigned.
