Job summary

Central Valley PACE in Modesto, CA is seeking a PACE Personal Care Assistant for our Day Center to support nursing staff by providing basic personal care and daily living activities for participants.

The position is Monday through Friday, 8:00am to 5:00pm. Benefited employees receive medical, dental, vision, life, AD&D, LTD, a 403(b) plan with employer match, a Roth option, plus a 401(a) after one year, FSA, and paid vacation and holidays.

Qualifications

• Valid California Driver’s License and reliable transportation.
• Ability to treat people of all ages including geriatrics.
• Meet CMS-approved competencies before working independently.
• Prior personal care assistance experience or willingness to complete training.

Responsibilities

• Provide basic personal care and activities of daily living.
• Communicate patient care needs to LVN/Supervising RN and Manager.
• Assist in Day Center activities when possible.
• Deliver DME to participants' homes and document in EMR.
• Observe and report changes in participants’ condition and care plans.
