Polaris Pharmacy Services in Columbus, OH seeks a Staff Pharmacist to manage daily dispensing duties in a long-term/post-acute setting. You will ensure label accuracy, maintain resident histories, and handle sterile IV products while adhering to HIPAA and regulatory standards.

The role requires an active Ohio pharmacist license, PharmD or equivalent, and experience in acute care; clinical collaboration and documentation are essential for patient safety and care quality.

Qualifications

• Current Pharmacist license active and in good standing in Ohio.
• Doctorate in Pharmacy or PharmD from an accredited School of Pharmacy.
• Experience in acute-care settings preferred.
• Management experience is a plus.
• Knowledge of intravenous admixtures services and therapy desirable.

Responsibilities

• Verify completeness and accuracy of all new and refill labels and orders.
• Maintain resident medical history and related data in the database.
• Assist in controlled substances handling and perpetual inventory.
• Reconcile controlled substances inventory per regulations.
• Provide clinical drug information to staff and patients.
• Document communications with physicians, nurses and others in records.
• Prepare and supervise sterile IV drug products or solutions.
• Provide evening, weekend, and holiday coverage as needed.
• Follow HIPAA and other regulations; perform other duties as assigned.
• Ensure workflow efficiency and appropriate outcomes.
