Job summary

Hartford HealthCare in Manchester, CT, seeks a Pharmacist to optimize drug therapy, supervise medication preparation, and provide drug information to the medical team. The role requires a PharmD and CT license.

The position supports cost-effective, patient-focused care on the 3rd shift (7-on 7-off) with EPIC experience preferred; PGY-1 residency and pharmacy teamwork are valued as part of a comprehensive healthcare network.

Qualifications

• PharmD degree required.
• Current CT Pharmacist license or eligible within 45 days of hire.
• PGY-1 Pharmacy residency preferred. EPIC systems experience preferred.

Responsibilities

• Optimizes drug therapy for the general patient population by designing therapy that is appropriate, correct and cost-effective to maximize outcomes.
• May provide direct patient care for pain management through evaluation, recommendations, and follow-ups in response to physician requests or critical paths.
• Maintains safety of drug therapy by using screening mechanisms and input from providers, patients and families to prevent adverse drug events.
• Provides drug information through participation on Health Care Team activities and as a liaison between the HCT and pharmacy for rapid, effective patient care.
• Investigates and resolves drug and pharmacy-related problems for immediate or broader review using investigative studies and problem-solving techniques for quality improvement.
• Documents interventions, outcomes, and statistics manually or via electronic documentation to show activity and support quality improvement.
• Demonstrates competencies to meet age-specific patient needs.
• Periodically may function as charge pharmacist.

Pharmacists Perform Functions Related To The Control, Monitoring And Dispensing Of Medications For All Age Groups Of Patients According To Department Procedures And Protocols In a Safe, Cost Effective And Timely Manner. Supervises Medication Preparation By Pharmacy Technicians And Advises Nursing Personnel Regarding Appropriate Medication Administration. Pharmacists Promote The Rational Use Of Medications Via Provision Of Drug Information And Clinical Monitoring Services To Members Of The Medical Community. Additionally Pharmacists Will

• Maintains safety of drug therapy for the general patient population by utilizing a variety of screening mechanisms and input from others (i.e. health care providers, the patient, and the patient's family) in order to identify, prevent and resolve adverse drug events.
• Provides useful drug information through participation on Health Care Team activities and as a liaison between the HCT and the pharmacy to facilitate rapid and effective patient care.
• Investigates and resolves drug and/or pharmacy related problems for immediate need or broader, longer term review by conducting investigative studies and/or utilizing effective problem solving techniques to ensure a high performing medication use system.
• Documents interventions, outcomes, and other statistics manually and/or through electronic documentation on a daily or sporadic basis to demonstrate activity and success and provide an opportunity for quality improvement.
• Demonstrates competencies, including knowledge and behaviors, to meet age-specific patient needs.
• Periodically may be required to function as charge pharmacist.
