Job summary

Community Care, Inc. Home Health is seeking aLicensed Practical Nurse for pediatric private duty (Trach/Vent) in Scottdale, PA. This full-time overnight role provides one-on-one care to a 16-year-old male child in a home setting.

Responsibilities include ADLs, g-tube care, vitals, seizures, trach/vent care, oxygen administration, and safe med delivery. Requires PA LPN/RN license, CPR, and reliable transport. PTO, insurance, and on-call support are offered.

Qualifications

• Current LPN (or RN) license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience (home health preferred).
• CPR certification required.
• Reliable transportation and a valid driver's license.
• A caring heart, independence, and pediatric focus.

Responsibilities

• Assist with Activities of Daily Living (ADLs).
• Perform g‑tube care/feedings/aspiration precautions.
• Monitor vitals.
• Monitor seizures.
• Trach/vent care and monitoring.
• Administer oxygen.
• Administer medications safely and accurately.
• Communicate clearly with the care team and family.
• Deliver family-centered pediatric care.
