Qualifications

• Must hold a New Jersey LPN license.
• Prior experience in long-term care or sub-acute settings preferred.
• Strong communication and interpersonal skills.
• 18 years or older and able to read/write/speak English.

Responsibilities

• Support resident care through ongoing assessment and observation.
• Admit, discharge, and transfer residents.
• Provide high-quality nursing care to residents per care plans.
• Maintain timely and accurate documentation of care and medications.
• Contribute to care planning, assessments, and evaluations.
