Qualifications

  • High School Diploma required.
  • Completion of state-approved CNA training program.
  • 6 months to 1 year experience working with youth with developmental or intellectual disabilities and/or complex medical needs (e.g., trach, G-tube, seizure protocols).
  • CPR/First Aid Certification.
  • Valid California driver's license, clean driving record and personal vehicle insurance acceptable to Hillsides' insurance carrier.

ESSENTIAL FUNCTIONS:

The essential functions include:

  • Provide direct supervision and support to Non-Minor Dependents (NMDs) 24/7 in a safe, structured, trauma-informed, and nurturing residential environment.
  • Build positive, trusting relationships with residents through consistent presence, engagement, and role modeling.
  • Implement individualized care plans, routines, and strategies developed by clinical, nursing, and program staff.
  • Coach and support residents in developing daily living and independence skills, including personal hygiene, meal preparation, money management, time management, housekeeping, and community navigation.
  • Assist residents in identifying and working toward personal goals related to independence, education, employment, and emotional wellness.
  • Support personal care needs such as bathing, grooming, oral care, dressing, toileting, continence care, and hygiene using adaptive techniques as needed and within scope of training.
  • Support mobility needs including safe transfers, walking assistance, repositioning, and use of mobility aids in accordance with training.
  • Assist with meals as needed, including feeding support, cutting food, monitoring intake, and observing for swallowing or choking concerns.
  • Monitor basic health indicators such as appetite, sleep, mood, elimination patterns, and vital signs (if trained), and promptly report concerns or changes to nursing staff.
  • Support the use, cleaning, and non-invasive maintenance of medical equipment (e.g., CPAP, oxygen, mobility devices) under appropriate supervision and training.
  • Accompany and support residents during medical, mental health, educational, and community appointments, including safe transportation using agency vehicles.
  • Facilitate and participate in structured recreational, educational, and social activities that promote regulation, skill-building, and positive peer interactions.
  • Engage residents in meaningful conversations and group discussions during meals, activities, and downtime to support connection and development.
  • Utilize positive behavior support strategies, coaching, and appropriate limit-setting to promote accountability, self-regulation, and socially appropriate behavior.
  • Monitor residents' emotional, behavioral, and physical well-being and respond appropriately to routine needs and crisis situations in accordance with agency protocols.
  • Collaborate as an active member of the multidisciplinary team, contributing observations and input to TILPs, NSPs, and ongoing care planning.
  • Maintain accurate, timely, and professional documentation including daily logs, observations, incidents, health-related information, and progress notes.
  • Support a clean, safe, and home-like living environment by guiding residents in housekeeping, organization, and safety practices.
  • Uphold confidentiality, professional boundaries, program policies, and ethical standards at all times.
  • Participate in required trainings, supervision, team meetings, and reflective practice to support ongoing professional development.
  • Demonstrate professionalism, reliability, cultural humility, emotional maturity, and respectful communication with residents, coworkers, and external partners.
  • Perform other related duties as assigned.
