Job summary

Kansas Health System is seeking an Clinical Pharmacist for the Inpatient unit at Olathe Hospital. This overnight role (9pm–7:30am, 7 on/7 off) focuses on safe, effective and cost-conscious drug therapy with strong interdisciplinary communication.

The pharmacist will participate in rounds, adjust dosing for renal/hepatic function, and lead educational activities. State licensure and board certifications are required or pursued as needed.

Qualifications

• Licensed Pharmacist – State Board of Pharmacy in state of primary practice site.
• ACLS, PALS/NRP as necessary per site requirements.
• BLS – American Heart Association.

Responsibilities

• Participates in performance improvement efforts to strengthen systems, policies and procedures.
• Attends patient care rounds with the multidisciplinary team and provides patient-focused pharmaceutical care.
• Supports staffing needs by flexing to other areas and covering supplemental shifts.
• Ensures timely medication order processing through entry, verification and problem resolution.
• Performs patient-specific medication reviews and adjusts dosing for organ dysfunction.
• Reports medication incidents and participates in safety reviews.
• Conducts medication reconciliation at admission, transfer, and discharge and educates patients.
• Educates staff through presentations and in-services; mentors students and interns.
