Job summary

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Direct Support: provide assistance per individual plans including socialization, health maintenance, medication administration, and daily living activities.
• Records: accurately document progress and review logs while maintaining confidentiality.
• Management of Individuals’ Assets: make cost-effective purchases within the individual’s budget and preferences.
• Relationships: maintain professional relationships with individuals, families, guardians, and case managers; uphold Sevita standards.
• Reporting: report abuse, medical, behavioral and other incidents as required; follow policies.
• Rights: protect confidentiality and assist individuals in exercising their rights.
