Job summary

Heart Havens seeks a caring Direct Support Professional to empower adults with developmental disabilities and help them live vibrant, independent lives. You will provide overnight supervision, assist with daily living activities, and support participation in community activities on weekends.

You will follow ISPs and health protocols, document care accurately, and maintain compliance with safety and policy guidelines while using Heart Havens’ values to guide daily interactions.

Qualifications

• High school diploma or GED preferred.
• 6 months’ direct support experience required.
• 1 year direct support in a DD residential group home preferred, or 2 years in DD with consistent job history.

Responsibilities

• Provides support to residents as outlined in ISP/Residential Plan for Supports.
• Supervises residents in program and never leaves them alone.
• Supports activities of daily living and encourages community engagement.
• Documents daily notes, ISP notes and T-Logs accurately.
• Administers medications per procedure and the 6 Rights.
