San Antonio Wellness and Rehabilitation Center is seeking a highly skilled Charge Nurse to lead our nursing team in a rehabilitation and skilled nursing setting. You will provide direct patient care, supervise staff, and coordinate with interdisciplinary teams to ensure high-quality outcomes.

This night-shift role (10PM–6AM) emphasizes leadership, accurate record-keeping, patient progress monitoring, and collaboration with physicians to implement effective care plans.

Responsibilities

• Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians
• Supervise and mentor nursing staff, providing guidance and support to ensure high-quality patient care
• Collaborate with interdisciplinary teams to develop and implement patient care plans, including setting goals and objectives
• Maintain accurate and timely records, including patient charts, medication records, and other required documentation
• Monitor patient progress, identifying and escalating concerns to address potential complications
• Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care
• Maintain a clean, safe, and organized environment, adhering to all infection control and safety protocols
