Job summary

San Antonio Wellness and Rehabilitation Center is seeking a Charge Nurse to lead our skilled nursing and rehabilitation team. The role covers the 2PM–10PM shift, focusing on safe, high‑quality resident care. Texas RN license and BLS certification, plus at least 2 years of nursing experience, are required.

You will supervise staff, coordinate care plans with physicians and therapists, monitor progress, and maintain accurate records in compliance with regulations.

Responsibilities

• Monitor and evaluate: continuously monitor patient progress, identifying areas for improvement and implementing changes to enhance the quality of care.
• Collaborate with interdisciplinary team: work closely with physical therapists, occupational therapists, speech therapists, and other healthcare professionals to ensure that patients receive comprehensive and coordinated care.

Job Title: Charge Nurse

Shift: 2PM - 10PM
