Ideal candidates are licensed RNs in Texas with at least 2 years of experience and BLS certification.

Job Title: Charge Nurse

Shift: 2PM - 10PM

About the Role:

As a Charge Nurse at San Antonio Wellness and Rehabilitation Center, you will play a pivotal role in leading our skilled nursing and rehabilitation team. Our center is dedicated to providing quality services that empower patients to reach their goals, and we are seeking a compassionate and experienced RN to join our team.

Responsibilities:

• Lead the nursing team: Supervise and coordinate the daily activities of the nursing staff, ensuring that patients receive high-quality care and services.

• Plan and implement care: Develop and implement individualized care plans, collaborating with physicians, therapists, and other healthcare professionals to achieve optimal patient outcomes.

• Monitor and evaluate: Continuously monitor patient progress, identifying areas for improvement and implementing changes to enhance the quality of care.

• Collaborate with interdisciplinary team: Work closely with physical therapists, occupational therapists, speech therapists, and other healthcare professionals to ensure that patients receive comprehensive and coordinated care.

• Maintain accurate records: Accurately document patient information, progress notes, and other relevant data to ensure compliance with regulatory requirements and internal policies.

Requirements:

• Current LVN or RN licensure: Must be a licensed Registered Nurse in the State of Texas.

• At least 2 years of experience:

• BLS certification: Must possess Basic Life Support (BLS) certification.

• Strong communication and leadership skills: Excellent interpersonal and communication skills, with the ability to lead and motivate a team.

• Adaptable and flexible: Ability to work in a fast-paced environment, with a focus on quality patient care and continuous improvement.
