Qualifications

• Experience in home health, hospice, nursing home, assisted living, or group home settings preferred.
• Knowledge of patient observation, medication administration, and vital signs monitoring required.
• Experience with memory care needs such as Alzheimer’s/dementia is highly desirable.
• Familiarity with care plans, HIPAA compliance, and patient confidentiality is essential.
• Certifications such as First Aid, CPR, or CNA are a plus but not mandatory; training may be provided.

Responsibilities

• Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
• Support patients with memory care needs such as Alzheimer’s or dementia through specialized techniques
• Operate medical equipment such as Hoyer lifts and monitor vital signs to ensure patient safety
• Maintain a clean and safe environment by performing light housekeeping tasks including cleaning and sanitation
• Provide emotional support and companionship to residents, fostering a caring and respectful atmosphere
• Follow HIPAA guidelines to ensure patient confidentiality and privacy at all times
• Collaborate with healthcare teams, social workers, and family members to deliver comprehensive care

Experience

• Prior experience in home health, hospice care, nursing home, assisted living, or group home settings preferred

• Experience working with individuals with developmental disabilities, autism, Alzheimer’s/dementia, or memory care needs is highly desirable
• Familiarity with care plans, patient monitoring techniques, and HIPAA compliance is essential
• Certifications such as First Aid, CPR, or Nursing Assistant (CNA) are a plus but not mandatory; training may be provided for qualified candidates
• Ability to perform heavy lifting when necessary and work effectively in both individual and team environments
