Job summary

Hilltop Community Resources Inc. in Grand Junction, CO is seeking a Care Specialist to support adults with traumatic brain injuries at the Life Adjustment Program.

This full-time role focuses on direct care, hygiene assistance, documentation, and behavior support within a values-driven team. The position offers full-time night schedules (10pm–8am) on Thu-Sun or Tue-Fri, with competitive pay ranging from $19.35–$20.31 per hour plus a $1.50 shift differential.

Qualifications

  • One year of related experience or two years of work experience required.
  • Minimum 18 years of age required; overnight roles may require 21.
  • Colorado Driver’s License preferred/required for overnight positions.
  • QMAP certification preferred/required for overnight positions.

Responsibilities

  • Assist and provide direct care and hygiene support per Care Plans.
  • Aid residents with meals, cueing, and supervision as needed.
  • Communicate needs of residents and staff for good customer service.
  • Complete required documentation promptly and accurately.
  • Provide basic behavior management and de-escalation.
  • Maintain professional boundaries and confidentiality.
  • Assist overnight residents with transportation using accessible vehicles if assigned.
  • Perform housekeeping duties to keep apartments and grounds clean.
  • Attend team meetings and trainings.

An overview of this position entails…This position is responsible for assisting and providing care support for adults with traumatic brain injuries at the Life Adjustment Program (LAP).

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Demonstrate ethical and professional behavior that align with the values of the Hilltop’s Life Adjustment Program, including Service 1, how we treat each other, and Service 2, how we treat the residents. Service 1 and Service 2 is demonstrated by setting boundaries, being predictable, consistent, kind, and transparent.

2. Providing direct, unskilled personal care and hygiene assistance to residents in accordance with their individualized Care Plans. This includes, but is not limited to, bathing, oral care, incontinence care ranging from minimal to severe, dressing, and grooming. Staff are also responsible for cleaning bodily fluids as necessary, which may include, but are not limited to, blood, urine, semen, feces, vomit, and other biological materials. Additionally, staff will assist and monitor residents during meals by offering support, supervision, and cueing as needed.

3. Properly communicating the needs of both residents and staff to ensure good customer service and the overall well-being of residents, natural supports, staff and vendors.

4. Completing required documentation promptly, accurately, and legibly.

5. Providing basic behavior management interventions (i.e., defusing anger, redirecting, offering alternative solutions and providing positive guidance for our residents) associated with characteristics of brain-injured individuals. Risks include sexual, verbal, and/or physical aggression that may be exacerbated by co-occurring mental illness, substance abuse, etc.

6. Maintaining professional boundaries with residents, co-workers, and outside agencies associated with any Hilltop program while maintaining the dignity, confidentiality, privacy and rights of every individual.

7. For Care Specialists assigned to overnight shifts, may be required to drive residents to and from offsite locations using handicapped-accessible vehicles, up to and including 12-person buses.

8. Assist in performing housekeeping duties as needed to maintain cleanliness of resident apartments and surrounding grounds. This may include, but not limited to, trash removal, cleaning up bodily fluids (i.e. blood, feces, urine, semen, vomit, etc.) while using the proper Personal Protective Equipment and disinfecting standards, disinfecting, cleaning wheelchairs and walkers, and organizing apartments.

9. Attending team meetings and in-service trainings as scheduled.

CERTIFICATES, LICENSES & REGISTRATIONS

  • Valid Colorado Driver’s License (preferred, required for overnight positions)
  • Qualified Medication Administration Personnel (QMAP) Certification (preferred, required for overnight positions)
