Job summary

Heritage Behavioral Health Center is hiring an overnight Urgent Care Clinician for our 24/7 Behavioral Health Urgent Care and Crisis Observation unit in Decatur. You will assess, de-escalate, and connect clients in crisis with trauma-informed care.

You’ll work with Recovery Support Specialists and supervision, with full training before independent coverage. On overnights you’ll complete full IM+CANS and ASAM assessments, plus crisis planning and discharge coordination, building strong clinical

Qualifications

• Bachelor’s degree in a human services field or related area with relevant supervised experience.
• Or a degree in any field plus 2 years of supervised clinical mental health experience.
• 5 years of documented, supervised mental health or substance use experience.
• DHS-approved Certificate of Psychiatric Rehabilitation with high school diploma or GED and 2 years of mental health work.

Responsibilities

• Greet and triage individuals in behavioral health crisis to determine acuity, safety risk, and next steps.
• Complete IM-CAT and a substance use screening for every individual.
• Provide crisis intervention, de-escalation, and brief treatment.
• Conduct suicide risk assessments and develop a diagnostic impression and disposition.
• Complete IM+CANS and ASAM assessments during overnight hours.
• Create a Crisis Safety and Prevention Plan with every individual before discharge.
• Reassess individuals in Crisis Observation at least every 4 hours and track the 23-hour window.
• Coordinate discharge, outpatient follow-up, or transfer to Crisis Stabilization Unit or inpatient care.
• Provide case management and linkage for high-risk individuals.
• Coordinate warm hand-offs with Mobile Crisis Response, Crisis Call Center, EDs, and partners.
• Complete timely, accurate documentation in the electronic health record.
