Are you passionate about making a difference in the lives of youth and families? Do you have the skills and heart to help families overcome challenges and achieve stability? We’re looking for a dedicated Outreach Worker to provide direct support to youth and families in need, guiding them on a path to success.

As an Outreach Worker, you will play a pivotal role in helping children and families achieve their goals and stabilize their lives. You’ll work in natural community settings, offering strength-based support and guiding youth in building essential life skills, like social skills and anger management. From providing transportation to facilitating access to crisis intervention services, your compassionate efforts will ensure families get the help they need, when they need it most.

Collaboration is key! You’ll work closely with clinical staff, participate in multidisciplinary team meetings, and support youth in expressing their voices during planning sessions. Your role will help bridge the gap between the challenges families face and the resources they need, all while making a real, tangible impact in the community.

Key Responsibilities:

  • Offers structured one-to-one strength-based and resiliency building activities to the identified child or youth. Services are provided in natural community-based settings.
  • Teaches social skills, anger management, and other related skills to youth.
  • Provides transportation as needed to address care coordination and treatment needs.
  • Supports family in accessing crisis intervention services as needed.
  • Closely collaborates with program clinical staff to ensure excellent coordination of care.
  • Participates in multidisciplinary team meetings and also works with all behavioral health providers involved with the child in order to support the goals of the service.
  • Coaches and supports the youth in expressing him/herself in these planning meetings.
  • Provides daily documentation of interactions with the identified individual served specific to the goal area identified in the treatment plan.


What We’re Looking For:

  • High School Diploma required. Bachelor’s Degree in the human service field, or in absence of Bachelor’s Degree five years related experience working with children and families required.
  • Individuals with lived experience in supporting a family member with mental illness or disabilities or with personal lived experience preferred.
  • Ability to create and implement strategies to facilitate achievement of agreed upon goals.
  • Valid driver’s license and reliable vehicle for all working hours required.
  • Demonstrates willingness and ability to work with client populations from diverse cultural and socioeconomic backgrounds.
  • Demonstrates working knowledge of community resources and how to help clients access them.
