Job summary

Behavioral Health Network in Western Massachusetts is seeking an Outreach Worker for the FIT program to deliver high-quality, community-based services to youth with Serious Emotional Disturbance and their families. You will work with a multidisciplinary team to provide intensive treatment, support, and skill building in home and community settings.

The role requires an Associate's or Bachelor's in Human Services (or enrollment in a Master's program), experience with youth and families, Spanish

Qualifications

• Associate's or Bachelor's degree in Human Services or related field; relevant experience or enrollment in Master's program.
• Experience providing therapeutic support to youth and families preferred.
• Knowledge of mental illness diagnoses, behavioral modification techniques, and coping strategies.
• Familiarity with child-serving systems and community resources.

Responsibilities

• Deliver intensive in-home and community-based services 3-5 times per week, as clinically indicated.
• Teach and coach youth in coping skills, self-care, and strategies for managing emotions.
• Support families in addressing the emotional and mental health needs of youth.
• Collaborate in the development and implementation of individualized treatment plans.
• Provide behavioral coaching to enhance problem-solving, communication, and family dynamics.
• Facilitate linkages to community resources and make referrals through warm hand-offs.
• Co-facilitate team meetings and assist with aftercare planning.

What You'll Do

As an Outreach Worker in the FIT program, you will provide high-quality, community-based services to youth with Serious Emotional Disturbance (SED) and their families.

You will work as part of a multidisciplinary team to deliver intensive treatment, support, and skill building to help stabilize youth in their home and community settings.

• Deliver intensive in-home and community-based services 3-5 times per week, as clinically indicated
• Teach and coach youth in coping skills, self-care, and strategies for managing emotions
• Support families in addressing the emotional and mental health needs of youth
• Collaborate in the development and implementation of individualized treatment plans

Who You Are

• Bilingual In Spanish (Required)

• Valid driver's license and reliable transportation required
