Job summary

Behavioral Health Network, Inc. is seeking an Outreach Worker in the FIT program to deliver intensive community-based services to youth with Serious Emotional Disturbance and their families.

You will work within a multidisciplinary team to provide treatment, support, and skill-building to stabilize youth at home and in the community. You will conduct in-home and community visits 3–5 times weekly, teach coping skills, support families, and participate in tailored treatment planning.

Qualifications

• Associate’s or Bachelor’s degree in Human Services or enrolled in a Master’s degree program.
• Experience providing therapeutic support to youth and families preferred.
• Knowledge of mental illness diagnoses, behavioral modification techniques, and coping strategies.
• Familiarity with child-serving systems and community resources.

Responsibilities

• Deliver intensive in-home and community-based services 3–5 times per week, as clinically indicated.
• Teach and coach youth in coping skills, self-care, and strategies for managing emotions.
• Support families in addressing the emotional and mental health needs of youth.
• Collaborate in the development and implementation of individualized treatment plans.
• Provide behavioral coaching to enhance problem-solving, communication, and family dynamics.
• Facilitate linkages to community resources and make referrals through warm hand-offs.
• Co-facilitate team meetings and assist with aftercare planning.

What You’ll Do

You will work as part of a multidisciplinary team to deliver intensive treatment, support, and skill building to help stabilize youth in their home and community settings.

Who You Are
