Job summary

Community Health Resources (CHR) is hiring an Outreach Therapist for the Assertive Community Treatment Team in Manchester, CT. This full-time position (40 hours/week) runs Mon-Fri 8:30am-5pm with one weeknight 12pm-8pm.

The role involves office, home, residential, and community-based settings per program needs. The candidate will assess and treat adults with serious mental illness, develop individualized plans, and coordinate care with families and other providers.

Qualifications

• Education: Master’s Degree or higher in related field.
• Experience: Experience with trauma, mental illness and/or substance use disorders preferred.
• Licensure/Certification/Registration: LMSW, LPC-A, LMFT-A required; LCSW, LMFT, LPC or Licensed Psychologist preferred; valid driver’s license where transportation is required.

Responsibilities

• Provides assessment and treatment interventions aligned with program model.
• Conducts clinical and risk assessments.
• Develops individualized, person-centered treatment plans with clients.
• Maintains client and staff records per agency policy.
• Provides crisis assessment and intervention and participates in supervision.
