Job summary

Community Health Resources is seeking an Outreach Therapist for the Intensive In-Home Child and Adolescent Psychiatric Service in Manchester, CT. This full-time role offers a flexible Monday–Friday schedule (8AM–8PM) with supervision and a supportive team, plus a company car and quarterly productivity bonuses.

Qualifications include a Master’s degree in a mental health field and licensure eligibility (LCSW/LMFT/LPC).

Qualifications

• Master's degree or higher in mental health field is required.
• Licensure as LCSW, LMFT, or LPC is preferred; eligible.
• Demonstrated experience as a psychotherapist with mental illness or serious emotional disorders.

Responsibilities

• Provides assessment and treatment interventions consistent with fidelity to evidence-based model.
• Documents ongoing client progress and outcomes.
• Demonstrates competency in age-related or specialty issues and developmental needs for each population served.
• Works with clients to develop appropriate discharge plans utilizing natural supports.
• Maintains collaborative relationships with family members and other service providers.
• Provides crisis assessment and intervention to monitor status of clients in crisis.
• Performs home and community-based care.
• Responsible for maintaining client and staff records according to agency policy.
• Provides coverage for team members during periods of absence.
• Participates in weekly supervision and meetings to review client and program progress.
