Job Summary

Under the general supervision of the Operations Manager, the Outreach Enrollment Specialist is primarily responsible for scheduling members for enrollment into an eligible care management program. The Outreach Enrollment Specialist supports the implementation and delivery of Sharecare’s VBCM program by providing high-quality telephonic/digital enrollment with members in a virtual call center environment. The VBCM program is based on a patient-centered model to help improve a member’s total well-being in an efficient, appropriate, and cost-effective manner. The position requires excellent sales, telephonic communication, and customer service skills.

Schedule

Monday - Friday, 8:30am – 5:00pm CT.

Position Start Date

June 2, 2025. Due to the structure of training, new hires are unable to miss any days for the first 30 days of employment.

Essential Job Functions
  • Utilizes automated dialer on internal telephony system to make outbound enrollment calls to members. Outbound calls are a primary responsibility with productivity and quality metrics determined by leadership.
  • Explains the VBCM program thoroughly and accurately, and schedules appointments between members and Care Managers using shared calendars.
  • Uses email to communicate confirmed appointments and modify them as needed.
  • Accurately collects, inputs, and updates data provided by members into the appropriate system in a timely manner.
  • Provides clerical support to the CF Value Based Care Management program as needed.
  • Cross-trains to support all team members administratively.
  • Completes necessary training to support the role.
  • Participates actively in team huddles and meetings as requested.
  • Processes data and maintains confidentiality of sensitive information.

  • Proficient with computers and familiar with Microsoft Outlook, Excel, and Word.
  • Effective with all management levels and colleagues, demonstrating initiative, judgment, and customer service excellence.

  • Organized, self-directed, capable of multitasking, and proficient in problem-solving.

Qualifications
