Seacoast Mental Health Center, Inc. seeks an Outreach Specialist/Case Manager for the Adult Services-Exeter, NH area. The role emphasizes coordinating services and providing direct support, advocacy and assertive outreach to adults with mental illness.

• One year of experience in the field is preferred.
• Excellent communication skills, both written and verbal; valid driver's license with clean driving record; CPR/First Aid training is recommended.

Responsabilités

• Coordinate services and provide direct support, advocacy and assertive outreach to adults with mental illness.
• Join the Adult Services Department in Exeter to support the community and improve client outcomes.
• Work collaboratively with the Exeter team to ensure case management and service coordination.
