Job summary

Qualifications

• Bachelor's degree in psychology or social work required; 1–2 years preferred.
• Valid driver's license with a clean driving record.
• Strong written and verbal communication skills.

Responsibilities

• Provide Functional Support Services to eligible adults and families in the community.
• Provide Targeted Case Management to assist adults under He-M 401 to access needed services.
• Deliver Illness Management and Recovery (IMR) services individually or in groups.
• Meet clinical performance standards set by the agency.
• Help build and maintain a natural support system with family, friends, and community partners.
• Maintain thorough documentation of contacts, assessments, and care plans.
